It takes place in online forums, in ophthalmology waiting rooms, and in candid exchanges between patients who achieved 20/20 vision on a Snellen chart yet find themselves struggling in ways they did not anticipate. For those considering laser correction, understanding this fuller picture is not a reason to abandon the idea — it is a reason to approach it with clear, realistic expectations.
The Gap Between Visual Acuity and Visual Quality
Optometrists and ophthalmologists are careful to distinguish between two related but distinct concepts: visual acuity and visual quality. Acuity refers to the sharpness of detail — what a patient can read on a standardized eye chart under controlled lighting. Quality, by contrast, encompasses how vision actually performs in the full range of real-world conditions: driving at night, reading a phone screen in a dim restaurant, watching a film in a darkened theater.

LASIK involves cutting a thin flap in the cornea's surface, a step that severs a substantial number of corneal nerves. These nerves are responsible not only for sensation but also for signaling the lacrimal glands to produce tears. The result, in many patients, is a temporary but sometimes prolonged disruption of the tear film. PRK, which removes the epithelial surface rather than creating a flap, carries a somewhat different risk profile but is not immune to this issue.
For some patients, post-surgical dry eye resolves within three to six months as nerve regeneration progresses. For others, it lingers considerably longer. A 2021 review in the journal Cornea noted that dry eye symptoms can persist in a subset of patients for a year or more, and in rare cases, may become a chronic condition.
Symptoms range from mild — occasional grittiness, sensitivity to air conditioning — to genuinely disruptive. Stinging, blurred vision that fluctuates throughout the day, and a paradoxical watering of the eyes (the body's overcompensating response to dryness) are all reported. Patients who already had subclinical dry eye before surgery, or who spent significant hours in front of screens, tended to experience more pronounced post-operative symptoms.

Regression, Undercorrection, and the Need for Enhancement

What Realistic Expectations Actually Look Like

The procedure has a strong overall safety record, and the majority of patients report high satisfaction when surveyed at the one-year mark. What the research and patient experiences collectively suggest, however, is that the conversation between surgeon and patient before surgery needs to be more substantive than a review of candidacy criteria alone.
Prospective patients are well-served by asking specific questions: What is the likelihood of nighttime visual disturbances given my pupil size and prescription? Do I have any pre-existing dry eye that could be exacerbated? What does the enhancement policy look like if my results regress? What is the realistic timeline for full visual stabilization?
It is also worth discussing lifestyle. A professional whose work requires extended nighttime driving, or someone who already manages chronic dry eye, may find that the tradeoffs of laser correction do not align well with their daily reality — even if they are technically eligible for the procedure.
Beyond candidacy, patients benefit from understanding that the recovery arc for both LASIK and PRK can extend well beyond the first few weeks. PRK in particular involves a surface-level corneal removal that typically requires three to five days of significant discomfort and several months before vision fully stabilizes. The early weeks can be discouraging, and patients who are not prepared for this sometimes interpret a normal healing process as a surgical failure.
